Terms and Conditions

GENERAL:

· Your dog must have basic obedience (sit, stay, come) and strong recall.

· Your dog must remain current with vaccinations.

· Your dog must be non-aggressive, and well socialized with people.

· Owners provide collars and leads (& coats if necessary). Canine Cardio is not responsible to replace damaged or lost collars, leashes, coats, registration tags, or name tags that may go missing or get damaged during walks.

Flea/Tick treatment and Kennel Cough immunization are recommended.

HOME SECURITY & DATA PROTECTION:

Canine Cardio will keep all keys, remote control entry devices, access codes, and personal information of the owner(s) secure and in safekeeping.

Canine Cardio will also return all the above at the end of the contract or immediately upon request.

COMMUNICATIONS:

Canine Cardio provides walk updates via text or email using Map My Walk app.

You must notify Canine Cardio if you would not like Canine Cardio to use your dog's photo in social media updates.

CANCELLATIONS:

12-hour notice is required to cancel or reschedule a walk/sitting.

In case of cancellation of a multi-night sitting, the first night fee is not refundable.

If Canine Cardio has been scheduled for a walk and arrives at a home without a dog present, you will be charged in full for that day's walk.

If unforeseen circumstances arise (outside of extreme weather conditions) where Canine Cardio would have to cancel a walk, Canine Cardio will reschedule that walk at no charge.

INCLEMENT WEATHER:

Walks take place rain, shine or snow (dangerous weather excluded). Canine Cardio will attempt to reschedule walks canceled due to possible dangerous weather conditions.

RETURNING DIRTY DOGS:

Due to the nature of dog walking on trails, dogs can get dirty. After a walk, dogs can be wet, dusty, or even muddy, so please be prepared to identify returning entrances during consultation. If your dog doesn't have a designated area or crating area, please leave a towel near the returning door/mudroom and Canine Cardio will do their best to wipe down your dog upon drop-off.

Please note: Canine Cardio is not a grooming service, so your dog will likely require cleaning in certain seasons. Feel free to ask Canine Cardio about professional grooming and we’ll be happy to help.

SAFETY:

Canine Cardio takes the safety of dog clients seriously. If your dog is deemed undisciplined, aggressive, or unresponsive the dog will not be accommodated by our services. If your dog does not exhibit these traits during the consultation, but they arise later, Canine Cardio withholds the option to terminate service immediately.

FLEAS & TICKS:

Ticks and fleas aren't foreign to dogs, particularly in warmer months. Canine Cardio recommends you check your dog regularly for both and requests that you make alternate arrangements to schedule walks if your dog is being treated to avoid spreading to other dogs.

KENNEL COUGH:

Kennel Cough is a Canine flu, if your dog is starting to cough, please let Canine Cardio immediately know. Kennel Cough (Bordetella) is very contagious. To prevent any possible spread to other clients, I will be required to change clothes at my place of residence so appointments may be cut short so as to not intrude on other clients’ time slots.

EMERGENCY PROCEDURE:

Canine Cardio carries a mobile phone for emergencies. If your dog is lost or hurt the protocol is as follows:

a) Contact emergency phone on client consultation sheet.

b) Contact Animal Services.

c) Return to the vehicle.

d) Attempt to locate the missing dog, or if injured, transport the dog to the nearest Veterinarian.

On-Leash Walks Waiver

I have read, understand, and acknowledged Canine Cardio’s terms and conditions and waiver by submitting the above form. By reading, understanding, and acknowledging the terms and conditions and waiver and proceeding into an agreement and schedule of care with Canine Cardio, I affirm that I understand the inherent risks and hazards associated with the nature of on-leash dog walking and Canine Cardio’s services. These risks also include, but are not limited to, risks associated with picking up my dog, dropping off my dog, traveling to and from my home in a vehicle, having my dog around other dogs, people, wildlife, and outdoor environments. Canine Cardio will take reasonable action to minimize these risks, but I understand it is not always possible to prevent injuries that may occur during on-leash walks in outdoor environments.
I, as the dog owner, am fully responsible for the actions of my dog if it causes any damage whatsoever, including, but not limited to, death, damage or injury to itself, or death, damage or injury to other animals, pets, people and property while in the care of Canine Cardio.
I affirm that I have provided all necessary information to Canine Cardio and through consultation deem that my dog is suitable to be in the care of Canine Cardio. I also affirm that I have not witnessed my dog display aggressive tendencies towards people or other animals in the past, or if so, have identified this in my submitted client contact form and/or during consultation. My dog is obedient and capable of following commands, is well behaved and has reliable recall even in distracting environments.
I agree to immediately advise Canine Cardio of any problems with my dog that could affect his/her behaviour, health, or suitability for on-leash walks.
In the event of any illness or injury to my dog while in the care of Canine Cardio, I authorize Canine Cardio to take my dog to the nearest veterinarian and agree to pay for all veterinary care required in the opinion of the veterinarian.
I agree to indemnify and hold harmless Canine Cardio against loss or threatened loss or expense by reason of the liability or potential liability of my dog for or arising out of any claims for damages. This includes, but is not limited to, medical attention to people, emergency, or veterinary care (including acts of nature, encounters with wild animals, etc.), any damages caused by acts of aggression (biting, attacks, etc.), or any injury caused by my dog toward property, animals and people while in the care of Canine Cardio. Additionally, any costs associated with the above actions will be wholly reimbursed to Canine Cardio upon request.
I affirm that my dog is spayed or neutered and up to date with the vaccinations. I understand that regular vaccinations cannot completely guard against illness and disease, and that Canine Cardio cannot prevent, nor be responsible for any illness my dog might contract. Canine Cardio shall not be liable for any illness or ailment which a dog may incur while in the presence of other dogs in Canine Cardio’s care.
I hereby waive and release Canine Cardio from any and all claims, actions, suits, demands, assessments or judgments asserted and any and all losses, liabilities, damages, costs and expenses (including, without limitation, attorneys’ fees and accounting fees to the extent permitted by law) alleged or incurred arising out of or relating to any operations.
My submission of the above consultation form confirms that I have completely read, understand, and acknowledged Canine Cardio’s waiver, Canine Cardio terms and conditions, and completed the Canine Cardio client information form and understand and agree to all of the terms outlined.

Dog Sitting Waiver

By accepting this document I, as the owner/agent/guardian, guarantee that I will be personally liable for all expenses resulting from daycare, boarding, veterinarian fees, property damage fees or any other expenses incurred while my dog/cat is visiting or being boarded with Canine Cardio.

I also understand that as the owner/agent/guardian I agree to indemnify and hold harmless Canine Cardio, its owners, agents and employees for any damaging acts to property or other animals while my pet is in transport or on the premise. Also, by signing this form I authorize any Canine Cardio contractor to seek immediate veterinary care for my dog/cat if deemed necessary by the agent and/or employee.

I understand that Canine Cardio Dog Sitting takes place at my home. I accept all risks involved and agree that Canine Cardio is not liable for any injuries or illnesses during their stay.

I further understand that due to the way that dogs/cats interact with one another, minor cuts and scratches can occur even though the dogs/cats are carefully supervised.

I also understand and agree that in taking care of my dog/cat, Canine Cardio is relying upon my representation and warranty that my dog/cat is in good health, and it has not harmed or shown aggressive or threatening behaviour towards any person or any other dogs/cats. By signing this form, I certify that all information provided by me in my application form is complete and accurate.

I represent and warrant that my dog/cat has not been ill with any communicable condition or otherwise in the last 30 days before his/her admittance with Canine Cardio. I also represent and warrant that all required vaccinations for the dog/cat are up to date.

Canine Cardio accepts no responsibility for any injury contracted or suffered while my dog/cat is being watched or boarded. I hereby release Canine Cardio from all liability for any loss or expense that I may suffer or bear as a result of any happenings (such as disappearance, runaway, sickness, injury, death from any cause, fire, theft etc.) through transportation or otherwise during the time that my dog/cat is in the custody and control of Canine Cardio.

Keys: (for our dog walking/cat visit customers) I understand Canine Cardio will obtain the key or the door code of my house.

I certify that I have read and understood the terms as set forth herein and that I have read and understood this agreement.

I agree to abide by all Canine Cardio policies and accept all the terms, conditions and statements of this agreement and confirm the truthfulness of the contents of the application form completed by me. I certify by signing this document that I am fully aware of all risks and that this document is considered legal and binding.
